By signing this form you agree that Smallwood should set up the desired maid services, as requested.  It is understood that Smallwood Plaza will bill the residents apartment for all the charges.  The standard cleaning fee is $100.00.  Anything over three (3) hours will be billed at a $25.00 per hour rate.  All charges are to be paid within five (5) days of receipt of notice of billing from Smallwood that the charge has been placed on your account.  Smallwood is authorized by the resident to allow the maid service employee access to your apartment to clean as you have requested if you are not home to do so.
